Benefit verification may not always be accurate. It is the client’s responsibility to follow up with their insurance provider for confirmation.

Your insurance provider can give you specific information about your plan, including your deductible, co-pays, and co-insurance.

Clients are responsible for any remaining balance if their insurance does not cover the services.

Rooted Collective does not bill out-of-network insurance plans. We are only able to submit claims to in-network insurances. However, we are happy to provide a Superbill for clients seeking out-of-network reimbursement.

Payments & co-pays are due the date of service.

If you are unable to attend your scheduled session, please notify me as soon as possible and no later than 24 hours before your appointment. Cancellations made with less than 24 hours’ notice, as well as no-shows, are subject to an $85 fee.

If you are running late, please let me know as soon as possible. Appointments are considered a no-show 15 minutes after the scheduled start time. If you know you will be more than 15 minutes late, please reach out as soon as you can so we can determine whether the session can still take place.
