Benefit verification may not always be accurate. It is the client’s responsibility to follow up with their insurance provider for confirmation.

Your insurance provider can give you specific information about your plan, including your deductible, co-pays, and co-insurance.

Clients are responsible for any remaining balance if their insurance does not cover the services.

Rooted Collective does not bill out-of-network insurance plans. We are only able to submit claims to in-network insurances. However, we are happy to provide a Superbill for clients seeking out-of-network reimbursement.

Payments & co-pays are due the date of service.

If you are unable to attend your scheduled session, please notify Ashley as soon as possible and no later than 24 hours in advance. Cancellations made within 24 hours of the appointment time, as well as missed appointments without notice, are subject to a $85 fee.
